ITV (Inspección Técnica de Vehículos) is the umbrella name for Spain's vehicle inspection system, but 'getting the ITV done' means something different depending on where a car is in its life. An imported vehicle typically encounters two separate inspection events, at different stages, checking different things.

Pre-registration ITV (ITV de matriculación)

This is the inspection an imported vehicle undergoes before it is registered in Spain for the first time. Its purpose is not primarily roadworthiness in the everyday sense — it verifies that the vehicle's technical characteristics (weight, dimensions, engine, emissions category) match what is being declared for registration, particularly important when there is no COC (Certificate of Conformity) to confirm this data automatically. It also assigns and verifies the technical file that will accompany the car under its new Spanish plate for the rest of its life in the country.

If your car comes with a valid COC, this step is often streamlined significantly, since the technical data is already certified by the manufacturer and accepted directly. Periodic ITV (ITV periódica)

Once a car is registered and circulating on Spanish plates, it enters the ordinary periodic inspection cycle that applies to every vehicle in Spain, imported or not. The schedule depends on the vehicle's age: newer cars typically go several years between their first periodic inspection, then shift to inspections every two years, and finally to annual inspections as they age further. This periodic ITV checks brakes, lights, emissions, tyres, structural condition and other everyday roadworthiness items — it has nothing to do with confirming the car's original technical declaration, which was already settled at pre-registration.
